Indications/Uses
DM in adults, adolescents & childn ≥1 yr.
Dosage/Direction for Use
SC Inj in thigh, upper arm or abdominal wall. Rotate inj sites w/in same region. Individualized dose. Administer once daily at any time of the day, preferably same time every day, or min of 8 hr between inj. Type 2 DM Monotherapy or in combination w/ oral antidiabetics, GLP-1 receptor agonists & bolus insulin: Initially 10 u followed by individual dose adjustments. Type 1 DM In combination w/ short-/rapid-acting insulin: Once daily w/ mealtime insulin & subsequent individual dose adjustments.
Special Precautions
Not to be administered IV, IM, or via insulin infusion pumps. Discontinue pioglitazone in combination therapy if any deterioration in cardiac symptoms occurs. Hypo-/hyperglycaemia. Omission of meal or unplanned strenuous physical exercise; high-dose insulin may lead to hypoglycaemia. Concomitant illness, especially infections & fever; kidney & liver diseases or diseases affecting adrenal, pituitary or thyroid gland. Usual warning symptoms may disappear in patients w/ long-standing diabetes. Prolonged effect may delay recovery from hypoglycemia. Inadequate dosing &/or discontinuation of treatment may lead to hyperglycaemia & potentially to diabetic ketoacidosis. Temporary worsening of diabetic retinopathy w/ intensification of insulin therapy. May form insulin Abs. Patients w/ risk factors for cardiac failure. Perform continuous inj site rotation to reduce risk of developing lipodystrophy & cutaneous amyloidosis. Monitor blood glucose & adjust dose after change in inj site from affected to unaffected area. Observe for signs & symptoms of heart failure, wt gain & oedema if used in combination w/ pioglitazones. May impair ability to drive & use machines. Monitor & control blood glucose in pregnant women w/ diabetes throughout pregnancy. Lactation. Childn <1 yr. Match insulin dose w/ food intake & physical activities in childn.
Adverse Reactions
Drug Interactions
Reduced insulin requirement w/ oral antidiabetics, GLP-1 receptor agonists, MAOIs, β-blockers, ACE inhibitors, salicylates, anabolic steroids & sulfonamides. Increased insulin requirement w/ OCs, thiazides, glucocorticoids, thyroid hormones, sympathomimetics, growth hormone & danazol. Masked hypoglycaemic symptoms w/ β-blockers. Increased or decreased insulin requirement w/ octreotide/lanreotide. Intensified or reduced hypoglycaemic effect w/ alcohol.
